Health, Dental, Vision, Life and other insurance programs are available to eligible employees as provided by Central Management Services and authorized managed care representatives. Human Resource Services provides this information as a resource for employees. Eligible employees are offered several options for life insurance coverage as provided by the program administered by the State of Illinois.
- Basic Life Insurance Coverage is provided at no cost to eligible active employees through the State of Illinois and Minnesota Life. The basic life coverage is equal to the annual base salary for active members.
- Optional Life Insurance Coverage is available to eligible active employees at their expense through the State of Illinois and Minnesota Life. Employees can elect coverage amounts up to eight (8) times their annual base salary. Optional life insurance elections may require a Statement of Health prior to approval.
- Accidental Death and Dismemberment is available to members at their expense through the State of Illinois and Minnesota Life. Employees can elect coverage amounts equal to their basic life coverage only, or an amount equal to the combined basic plus optional life insurance amount not exceeding five (5) times basic life amounts. Acceptance to the plan is guaranteed.
- Spouse and child life insurance coverage is available to members at their expense through of the State of Illinois and Minnesota Life. Spouse and child life insurance coverage amounts are equal to $10,000 per dependent. Spouse and child life insurance elections may require a Statement of Health prior to approval.
