Flu Shots

Annual on-campus flu shots are being offered by the DeKalb County Health Department, NIU School of Nursing and the Employee Assistance Program (EAP). The flu shot is paid for by the employee's state health insurance provider for only NIU employees and retirees. Employees who are dependents, or employees who opted out of the state insurance, dependents, spouses or partners who are not NIU employees are charged a fee for their flu shot. Students can get their flu shot at NIU Health Service for a reduced rate.

Flu Shot Forms

Note: Please make sure to complete the Administration Record prior to, and bring with you when come for your Flu shot. This helps the DeKalb County Health Department complete their Emergency Preparedness Training.

If you are unable to attend the campus clinics, the flu shot may be obtained at the DeKalb County Health Department, 2550 N Annie Glidden Road, DeKalb, IL. Monday through Friday from 8:30am to 11:30am and 1:00pm to 4:00pm. For more information, call 815-748-2467. If you receive your vaccine at the DeKalb County Health Department. State ID and insurance cards will be required. Flu vaccine must be administered prior to December 31, 2017 in order to be paid by the State of Illinois.
