University offices will reopen on Wednesday, January 2, 2002. Please take this announcement home with you for easy reference during the holiday season. HOSPITALIZATION AND/OR SURGERY
Quality Care Plan (Unicare) - Requires precertification by calling 1-800-327-7443. Failure to give this notification on a timely basis can result in a $400 penalty to you. In case of planned care, call as soon as you are aware of the need for hospitalization or surgery. For emergency admissions, the precertification number must be called within 48 hours of admission.
Health Maintenance Organizations (HMO's) - Always contact your Primary Care Physician first. In case of an out-of-area emergency, call your Primary Care Physician as soon as possible.
VISION CARE
If you wish to obtain an eye exam (allowed every year), glasses or contacts (allowed every 2 years) under the Vision Service Plan (VSP) call your participating eye care physician and indicate that you have the VSP Plan when you make an appointment, and your physician's office will obtain the form for you.
COUNSELING, MENTAL HEALTH OR SUBSTANCE ABUSE SERVICES (Inpatient or outpatient)
o Quality Care Program: Call Magellan at 1-800-513-2611.
o Health Alliance (DeKalb Clinic HMO): Call your Primary Care Physician at DeKalb Clinic - 815/758-8671.
o Other HMO's: Follow the procedures established by your HMO, or Contact your Primary Care Physician.

HOLIDAY PAYROLL INFORMATION
December 14, 2001
All payments (direct deposit and printed paychecks) will be distributed on Friday, December 14. Printed paychecks will be delivered to all departments by Campus Mail Services on that same date.
December 31, 2001
Direct deposits will be credited to employee accounts on Monday, December 31. University issued paychecks will not be available for pick up by departmental representatives at Campus Mail Services on December 31. Paychecks will be delivered to university departments with the regular campus mail delivery on January 2, 2002. As an alternative, departments may arrange to pick up and distribute printed paychecks between 7:00 a.m. and 8:00 a.m. on Monday, December 31. Departments arranging to receive paychecks on December 31 will be required to recognize specific paycheck distribution procedures.
