Faculty and SPS Employees on Academic Year Contracts
Human Resource Services would like to remind employees who may be off payroll during the upcoming academic break that this constitutes a qualifying event and allows certain changes to be made to insurance benefits.
Employees will have 60 days from the beginning of their academic break to request a change or waiver of coverage by completing the Leave of Absence Continuation or Waiver of Coverage Form. Completed forms should be returned by campus mail, United States Postal Service or fax to:

The effective date of a benefit change or waiver will be the date the form is received in Human Resources or the date the employee was off payroll, whichever is later. If there has been any premium paid, there will be no refund of these premiums. Human Resource Services will continue your current coverage unless a form is received requesting a change or waiver.
