Please take this announcement home with you for easy reference during the holiday season. Remember, you can access any benefit form and benefits information on the Human Resource Services web site, www.hr.niu.edu under “Forms.” The HRS web site also contains links to benefit provider web sites.
HOSPITALIZATION AND/OR SURGERY
Quality Care Plan (Cigna) – Requires precertification by calling 1-800-962-0051. Failure to complete certification on a timely basis can result in assessment of an $800 penalty. In case of planned care, call as soon as you are aware of the need for hospitalization or surgery. For emergency admissions, the precertification number must be called within 48 hours of admission.
Health Maintenance Organizations (HMOs) – Always contact your primary care physician first. In case of an out-of-area emergency, call your primary care physician as soon as possible.
VISION CARE
If you wish to obtain an eye exam (allowed every year) glasses or contacts (allowed every 2 years) under the EyeMed Vision Plan, call your participating vision care physician and indicate that you have EyeMed when you make an appointment, and the vision care provider’s office will obtain the form for you.

Holiday Payroll Information
Direct deposits will be credited to employee accounts on Monday, December 31. Printed paychecks will be sent to your home address. NIU cannot guarantee U.S. mail delivery times. However, paychecks will be mailed early on payday in order to arrive at your currently listed home address in the timeliest manner possible.
If you have any questions concerning holiday payroll distributions, or if you do not wish to have your check sent by U.S. mail, please contact Kathy Smith at 753-3601 or by email at klsmith@niu.edu. Subject to notification being provided to Kathy Smith prior to December 20, 2007, your paycheck can be made available for pickup at the Swen Parson Payroll Distribution Center on January 2 from 8:00 a.m. to 10:00 a.m.
- Hourly Time Entry summaries for the period 12/16 through 12/31 are due January 2, 2008.
