SEASONAL FLU SHOTS 2009

The DeKalb County Health Department, NIU Health Services,
and Employee Assistance Program will provide flu shots at the
NIU HOLMES STUDENT CENTER BALLROOM

Tuesday, September 22 10:00 a.m. to 2:00 p.m.
Wednesday, October 14 11:00 a.m. to 1:00 p.m.

NO APPOINTMENT IS NECESSARY

  • You must present your state health insurance card and state employee identification card to receive a free flu shot. You are also required to provide the last four digits of your Social Security Number for identification and payment purposes.
  • Flu shots will be free to any full- or part-time staff and faculty who have health care coverage through one of the State of Illinois Health Care Plans including the HMO. The State of Illinois Health Care Plans do not cover graduate students with assistantships.
  • Retired employees and survivors with State of Illinois health care coverage, not yet eligible for Medicare, are also eligible for the free flu shots.
  • Only employees (not their covered adult dependents) will be eligible for the free flu shot. However, adult dependents and others may receive the shot by paying the $37 fee. Flu shots for children under the age of 19 must be received at the DeKalb County Health Department.
  • Medicare will be billed for individuals with Medicare Part B medical coverage. You must bring your Medicare card the day of the shots.
  • Shots must be received before January 14, 2010. After that date, you will be required to pay $37.
  • Students may receive their flu shots at NIU Health Services for $20 or from the DeKalb County Health Department for $37.

Remember to dress so that the nurse can easily access your upper arm. Vaccine will not be given to individuals allergic to eggs or who report a history of previous adverse reactions from the influenza vaccine.

If you are unable to attend these clinics, the flu shot may be obtained for adults 19 years and older at the public walk-in clinics held at the DeKalb County Health Department, 2550 N. Annie Glidden Road, DeKalb, Illinois. Dates to be announced.

If you attend one of the other clinics, please tell the registration desk you are an NIU employee, retiree or survivor with health care coverage through one of the State of Illinois Health Care Plans to receive the vaccine free of charge.
