Beneficiary information for life insurance and retirement system benefits.
Name, address, date of birth and Social Security number.
Bank routing number and account number for direct deposit.
Dependent information for health insurance.
Name, date of birth, Social Security number and physician’s name (if selecting an HMO).
Current group health insurancecoverage.
Name of spouse’s employer, name and address of spouse’s insurance carrier, effective date of coverage, group policy number, and whether you are covered under your spouse’s policy.
Income tax information.
Filing status and number of exemptions for federal and state withholding.
